New patient overview

Preparing for your first appointment

Once your appointment is scheduled, our new patient coordinator will provide you with a welcome email with a Patient Portal login and a checklist of required forms. Please complete and return the forms included in your welcome email prior to your visit. These forms include:

  • New Patient Checklist and Forms

  • Carrier Screening

  • Email / SMS Correspondence Consent

  • Medical Records Release Request

  • IDS Patient Notice

  • Financial Policy

  • HIPAA Notice of Privacy Practices

To ensure productive consultation, please provide detailed information about your medical and infertility history before your visit our Patient Portal. Our doctors and care team will thoroughly review your case in advance to start developing a diagnosis and fertility treatment plan. As a courtesy, we can request your records, though processing may take 7-10 business days. 

We will also verify your insurance benefits and provide an outline of your coverage during your consultation. If you have coverage, we will bill your insurance and only collect your co-payment. If not, a discounted cash fee will be charged for your consultation. Our New Patient Coordinator will review our fees at the time you schedule your Zoom appointment. The discounted Zoom consultation fee is $230.

  • When you schedule your New Patient Consultation, you will receive a User ID and Password. Please click the Green Patient Portal Button to log into the portal and complete your medical history and demographic information.

    Important Notes:

    • Do NOT click on “Request New User ID and Password.” Use the credentials provided to you at the time of scheduling.

    • The portal must be completed before your scheduled appointment date.

    Steps to Complete the Patient Portal:

    1. Log into the portal using your assigned User ID and Password.

    2. Review and agree to the terms and conditions to proceed.

    3. Update your demographic information on the Demographic Screen and click “Update.”

    4. Locate the History Form Tab (orange tab in the upper left corner) and complete the form in full. This step is mandatory and must be finished before your appointment.

  • Please arrive 15 minutes early to keep appointments on schedule. We will confirm your appointment at least two days prior via phone or email. A $100 fee applies for cancellations or rescheduling within 24 hours.

    Out of respect for other patients, please arrange alternative childcare during office visits.
